It sometimes happens if a datapack has not been erased for long enough in a UV eraser, or if the pack is corrupted (many ways possible, but more often than not poor contacts).
I assume the pack was formatted.
If it was a pack you bought from me (Lepowerfulpierre on eBay), I will replace it free of charge.

I think the issue is I need to setup the software before I connect the datapak. The XP is initialising Pak when I need it blank for the image to be written to it.

Check out the section - 3 MAKE and AMAKE - to get the 'insert pack now' procedure right.
 
Are you using a COM1 / COM2 Comms Link, or a USB one?

If the Datapak is OK, it is normal to wait for the software to tell you when to insert it.  The Organiser will try to format a blank pack, and if the software is expecting a blank one... so just wait for the software to tell you to insert the pack.
